Is ANYONE else having absolutely UNHINGED dreams on semaglutide?? I'm talking vivid, cinematic, emotionally intense dreams that I remember in full detail when I wake up. Last night I dreamed I was conducting a symphony orchestra made entirely of cats. The night before I was on trial for stealing the moon. WHAT IS HAPPENING.
I never remembered my dreams before sema. Now I have a full movie every single night. My husband says I've been talking in my sleep too, which I never did before.
This started about 3 weeks into treatment (currently on 1mg, month 3). Anyone else? Am I losing it? 😂
YES. Oh my god YES. I thought I was going crazy. The dreams are INSANE. Last week I dreamed I was dating Ryan Reynolds but he was also somehow a librarian who lived in a treehouse. It was SO REAL. I woke up genuinely confused that I was in my own bed.
I started googling "semaglutide vivid dreams" and found TONS of reports. It's definitely a thing.
The sema dreams are absolutely a widespread phenomenon even though they're not well-captured in the clinical trial data. They weren't a tracked adverse event in the STEP trials so we don't have great prevalence data, but patient forums and post-marketing surveys suggest it's common.
The proposed mechanisms:
- GLP-1 receptors in the brain: GLP-1R is expressed in the hypothalamus, brainstem, and — importantly — the hippocampus, which is involved in memory consolidation during sleep. Semaglutide crosses the blood-brain barrier. It may be modulating sleep architecture and REM cycles.[1]
- Blood sugar changes: More stable overnight glucose levels (especially in people with insulin resistance) can change sleep quality and dream recall. Hypoglycemia is known to increase REM sleep and vivid dreams.
- Weight loss itself: Significant weight loss improves sleep apnea, which increases time in REM sleep, which increases dream recall.
- Dietary changes: Eating less food, especially less food close to bedtime, can alter sleep architecture.
Most likely it's a combination of all of the above. The cat orchestra dreams are probably just your brain having fun with the extra REM time 😄

Ok but can we talk about the EMOTIONAL dreams? I had a dream last night that my grandmother (who passed 10 years ago) was sitting in my kitchen making pancakes and we had this long conversation about her life. I woke up sobbing. Like, full body sobbing. These aren't just vivid dreams — they're emotionally overwhelming in a way I've never experienced.
